Open Bed at the Group Home - APLA Serra Project Long Beach
The Serra Project Long Beach Residential Care Facility for the Chronically III (RCFCI) Group Home Program is a State-Licensed, DHSP Funded program designed to serve persons living with HIV/AIDS who are unable to perform activities of daily living to meet their personal needs. The program provides nutritious meals and 24/7 support by Certified Nursing Assistants, RN Case Managers, MSW, and an LCSW.
- MEDICAL ELIGIBILITY -
* HIV/AIDS Diagnosis (*Current form with CD4 & VL)
* Karnofsky Score of 70% or below.
* Age 18 or older

Mandatory Requirements: Negative COVID-19 test at least 24-48 hours prior to scheduled admission.
One open bed available at the group home located on the 2nd floor. Client must be able to walk up and down the stairs independently.
